We are Georgia Health Services Network, a licensed Medicaid Provider for the State of Georgia. We implement Georgia Medicaid’s SOURCE and CCSP Programs in licensed personal care homes throughout Georgia.
We help families overcome the financial burden associated with assisted living and work closely with the medical community (Social Workers, Case Managers, RN’s, etc) in finding affordable housing for individuals from the low and middle-income groups in need of assisted living. There is not fee for our placement services. We facilitate placements for Clients who qualify for Medicaid as well as Private Pay Clients.
Once our agency places a client in one of our contracted Licensed Personal Care Homes, our Nursing Staff begins the care coordination/case management component of the CCSP & SOURCE Program.
