Georgia Health Services works closely with social workers, case managers, nurses, doctors, etc in finding affordable housing for medically dependent people.
Call us and let us help you with your discharge planning needs. Our agency helps prospective clients enroll in Georgia Medicaid Waiver programs such as CCSP or SOURCE.
Our placement coordination services are Free to the clients and their families!!
If you are working with a person in need of a personal care home call us at (770) 466-7771 or Contact Us via email with the referral information.
If would prefer to fax in the referral information please print our Referral Sheet and fax it to (770) 466-3810. A placement coordinator will contact you within 24 hrs regarding placement options into a personal care home and determine if the client is eligible for either the SOURCE or CCSP Programs.
Please note: All clients being admitted into a licensed personal care home are required to have a Physician Medical Evaluation Form and a negative TB Test or clear Chest x-ray. This Physician Medical Evaluation Form must be completed by a licensed physician prior to admittance into a licensed personal care home.
