Care Coordination is done by the Registered Nurses employed by Georgia Health Services to monitor the Personal Care Homes. Our Nursing Staff supervises the care the clients receive in the Licensed Personal Care Homes. Clients on the CCSP or SOURCE programs receive visits from the RN  twice per month, at least 14 days apart.  This level of monitoring helps ensure the health and well being of our clients on the Programs.

Clients under the ICWP, CCSP and SOURCE program have the services of the Nursing staff to help coordinate referrals with various Medical Specialists(Cardiologists, Pulmonologists, Neurologists) as well as assistance with dealing with prescription issues. Nurses  review the client’s Medication Assistance Record and supervise care in the Personal Care Home. This medication monitoring helps detect if a client is  over-medicated or if a medication might be contraindicated for the client.
